For each question, assign a score based on the responses:

  • (A) = 0 points
  • (B) = 1 point
  • (C) = 2 points
  • (D) = 3 points

Add up the scores for all five questions. The total score provides an indication of severity:

  1. 0–3 points: Minimal
    • Your responses suggest you may not experience significant symptoms.
  2. 4–7 points: Mild
    • You might occasionally feel anxious, but it may not significantly interfere with daily life.
  3. 8–12 points: Moderate
    • Your anxiety may be affecting your daily life, and addressing it might help improve your well-being.
  4. 13–15 points: Severe
    • Anxiety is likely having a significant impact on your life. Consider seeking professional support.
